If you live in our practice area ( see the map in the practice area section) and would like to register with us, please complete one of the registration forms which can be obtained from reception or printed off from below. You will be required to provide ID (a passport of birth certificate), plus photo ID if a passport is not available as well as proof of address dated in the last 3 months. You will also need to complete the online medical questionnaire available below.
You will be registering with the practice not an individual GP, but are welcome to specify a preferred doctor if you wish.

Once you have completed these forms you will need to attend the surgery with the two forms plus the supporting documentation. Unfortunately we are unable to fully register you until all of the above stages have been completed. Please note it may take several months for your medical notes to be transferred to the practice.
Supporting documents are required to confirm your details (for each individual over 18 years). Children under 18 years old only require a copy of their birth certificate. One piece of evidence must be provided from each of the two sections below. One of which must confirm your address is within the practice area.
Passport or Birth Certificate. If a passport is not available then another form of photo ID will be required alongside a birth certificate.
Proof of address dated in the last three months
For patients requesting to register who are not ordinarily resident in the UK, please provide a European Health Insurance Card, S1 Form or work permit. Patients from outside the European Economic Area can be registered at the practice as a temporary patient but may be charged for healthcare within other areas of the NHS.
